Scheduling an Appointment

To schedule an appointment, please call:
(507) 247-5521 or (800) 644-6813.

Our staff will request the following information when you call to make an appointment with a doctor or physician's assistant:

• Your preferred provider (if you have visited us before)
• The reason for your visit

For New Patients

If you are a new patient at Tyler Healthcare Center/Avera, you will be asked to bring the following to your first visit:

• Insurance information
• Your most recent medical records

Canceling an Appointment

If you find it necessary to cancel an appointment, please do so at least 48 hours in advance. When canceling, we will request the following information:

• Name of your doctor
• Date and time of the appointment you wish to cancel

Please call 911 in case of an emergency, or proceed directly to emergency department.
