How to Become a Participating Provider
Thank you for your interest in becoming a participating provider with Avera Health Plans (AHP). Please select the option that best applies to your situation.
Participating Provider
(i.e. MD, DO, PA, Advanced Registered Nurse Practitioner, Masters Level Behavioral Health Provider, Psychologist, Doctor of Optometry, Physical Therapist, Occupational Therapist, Doctor of Podiatric Medicine, Speech Therapist, Audiologist and Durable Medical Equipment providers)
If you are new to AHP and do not have a contract in place, or if you already have a contract but need a provider application, you can request this information by emailing the Network Services Department at providers@averahealthplans.com or by phone Monday through Friday, 8 a.m. to 5 p.m. CT at (605) 322-4545 or toll-free at 1 (888) 322-2115.
If you already have a provider application, please complete and attach the necessary documents and mail it to:

Participating Facility
(i.e. Hospital, Home Health Agency, Skilled Nursing Facility, Free Standing Surgical Center or Behavioral Health Facility with inpatient settings)
